Note: Instrument sharpen, retip, repair requests have a 10 item minimum

1. Print a handpiece repair work order or instrument repair request form located on this page.

2. Complete the form with as much information as possible. Be sure to provide the following information:

   • Practice or Dentist Name

   • Contact information; please include a contact person and email address.

   • The handpiece/instrument details (type, make, model, serial number) as applicable.

   • Describe the nature of the problem.

   • Please indicate if you are requesting an estimate or repair for handpieces.

3. Package the handpiece(s) or instruments in one of our boxes (if you do not have one of our boxes you can use any small box or mailer you have available).

4. Affix our pre-paid shipping label to the package. If you do not already have one you can use the link provided on this page to create and print one.

5. Give the package to your regular mail carrier.
